Clinical Observations in Nepal
In our Kathmandu clinic, Syphilis is frequently diagnosed at the secondary or latent stage. Many patients come only after a positive blood test done for visa, marriage, or HIV screening, without remembering any genital sore.
- Early treatment with Benzathine Penicillin prevents long-term complications like neurosyphilis and heart disease.
- Co-infection with HIV is not uncommon in Nepal, so combined screening is strongly recommended.
- Many patients have taken incomplete antibiotics from pharmacies, which can change symptoms but does not cure the infection fully.
Treatment Protocols
Syphilis is treated according to stage. For early Syphilis (infection within 1 year), a single intramuscular dose of Benzathine Penicillin G 2.4 million units is used. For late latent or unknown duration, three weekly doses are required. The same medicine is available in Nepal and used as per international guidelines.

Early Syphilis (Primary, Secondary, Early Latent)
Single intramuscular dose of Benzathine Penicillin G 2.4 million units. Abstain from sex for at least 7 days after treatment and until partners are treated.
Late Latent or Unknown Duration
Benzathine Penicillin G 2.4 million units intramuscular once weekly for 3 weeks (total 3 doses). Follow-up RPR testing at 6, 12, and 24 months.
Penicillin Allergy Options
Doxycycline 100mg twice daily for 14-28 days may be used in some cases, but requires strict adherence and close follow-up. Penicillin desensitization is preferred in pregnancy.
Neurosyphilis and Eye Syphilis
These require intravenous Penicillin in hospital under specialist care. We arrange referral and coordinate with neurology and ophthalmology teams when needed.
Follow-up Schedule and Blood Tests
After treatment, regular blood tests (RPR or VDRL) are essential to confirm cure. In Nepal, these tests are available in most major labs. We usually repeat tests at 3, 6, and 12 months, and sometimes longer for late Syphilis.

For early Syphilis, we expect at least a four-fold drop in RPR/VDRL titer within 12 months (for example, from 1:32 to 1:8).
For late Syphilis, the fall in titer is slower, but it should still gradually decline over 24 months.
Some patients remain “serofast” with a low positive titer despite adequate treatment. In such cases, the trend over time and clinical evaluation are more important than a completely negative report.
